I am easily accessed from Routes 70, 73, and 38 and I am only one mile from the Cherry Hill exit off of Route 295 via Route 70 West. I am in the 1101 Building in Suite 302. The building has an elevator and is handicapped accessible.

My Office Manager is Ms. Karen Whirledge. If you call for an initial appointment, they may ask you for some brief clinical information to insure that I work with your particular type of problem and that we have mutually available times in which to meet each other. Your insurance information will also be collected to insure that I am in your network before you come to your first appointment. Confidentiality is always maintained.
You are encouraged to use this site to access, download, and complete all patient registration, HIPAA forms, medical/medication history, and for children under age 18, a Developmental History Questionnaire. If you are seeking a neuropsychological evaluation, you will also be asked to complete a History of Problems. Completing these ahead of time and bringing them to your first appointment. Map it.
If you are presently covered by insurance and the service that you need is a covered service, my office will work with you to enable you to access your full benefits.
It will be your responsibility to inform us of any and all changes to your insurance policy, coverage changes, copay changes, job changes, and address and phone number changes. My office manager, Ms. Karen Whirledge, spends much of her time and does everything humanly possible to streamline your time with us around insurance coverage and payment issues. However, we depend on you to provide us timely information about changes in your situation.
It is a contractual obligation for me to collect and for you to pay your copay and deductible amounts at the time of service. This is not negotiable. From time to time we encounter insurance payment problems and may call upon you to follow through on your end to make sure that appropriate and timely payments are made by your insurance company.
Please remember, the insurance coverage is yours, it covers you and/or your family for covered services that are rendered by me. Responsibility for payment is ultimately yours.
